Who we are
Elation Dental is operated by WMK Nasa Pty Ltd as trustee for the WMK Nasa Service Trust, at 111 Bayswater Rd, Croydon South VIC 3136. We handle personal information in line with the Privacy Act 1988 (Cth) and the Australian Privacy Principles, and health information in line with the Health Records Act 2001 (Vic).
What we collect
Your name, contact details, date of birth, Medicare and health fund details, medical and dental history, x-rays, photos and treatment records, and payment information.
Why we collect it
To provide safe dental care, manage appointments, process payments and health fund claims, and communicate with you, including appointment reminders and recalls.
Who we share it with
Only as needed for your care or as required by law: for example, specialists or other practitioners we refer you to, dental laboratories, your health fund, and Medicare. We do not sell your information.
How we store it
Records are kept securely in our practice management system (Dental4Windows) with access limited to authorised staff. We keep records for the periods required by law.
Our website
Our website may use cookies and analytics to understand how it is used. Information sent through our contact form is used only to respond to your enquiry. Please do not send detailed health information through the form.
Access and correction
You can ask to see or correct the information we hold about you. Contact our Practice Manager and we will respond within a reasonable time.
